Memory Clinics

Center for Senior Health and Longevity

The Center for Senior Health and Longevity is affiliated with the University of Wisconsin School of Medicine's Milwaukee Clinical Campus at Aurora Sinai Medical Center. Primary care and consultation are provided by faculty geriatricians and other medical professionals who specialize in working with older adults. Nurse clinicians and practitioners, case managers, therapists, a social worker and a psychologist make up the team of professionals.

 

Services include: Comprehensive geriatric assessment; Primary care; Case management; Social work and assistance; Continence evaluation and referral; Dementia evaluation clinic; Falls prevention; Community wellness center, Geriatric psychiatry; Research and education; Physical therapy/rehabilitation/evaluation; Hearing loss evaluation; ACE unit (acute care for elderly); GEM program (geriatric evaluation and management); Consultation service for physicians. Payment-all insurances accepted. Non english services provided-Russian and Spanish always available. Spanish speaking physician available. Other languages need advanced notice for interpretation.

This clinic is a member of the WAI-Affiliated Dementia Diagnostic Clinic Network. Network clinics differ from other memory clinics in that their staff have received training/observation time and/or guidance from the WAI and the UW Health Memory Assessment Clinic. In addition, clinic network staff attend the WAI's annual conference and bi-annual clinic network meetings, which provide up-to-date research and disease management information. Clinics in the network do remain autonomous, but follow a set of guidelines.
