Why would you like to participate in the Alzheimer’s Disease Summer Externship?

What specific medical activities and procedures do you hope to participate in or observe during the externship?

What ancillary activities of a physician (community involvement, nursing home visits) do you hope to experience during the externship?

What do you hope to get out of this experience for your personal and professional development?

The externship involves travel between Madison (1 week), Milwaukee (1 week), Richland Center (3 weeks) and Fort Atkinson/Whitewater/Johnson Creek (3 weeks). We will provide housing as needed to minimize your travel. Do you have a vehicle to get you to these locations? Will you need housing in any of these locations?

Do you have special interests you would like us to be aware of?

Is there anything else you would like to share with us?

What is your experience with the elderly? People with Alzheimer's disease?

The Wisconsin Alzheimer's Institute, University of Wisconsin School of Medicine and Public Health, under the direction of Mark Sager, MD, developed this opportunity and will provide mentorship and guidance during the externship. For more information contact Heidi Pophal, hpophal@wisc.edu or (608) 829-3312, Wisconsin Alzheimer’s Institute, University of Wisconsin School of Medicine and Public Health. If you experience difficulties filling out this form, please contact Heidi and she will mail you the application form.

The Urgency of Our Mission

In the U.S. approximately 5-10% of the population aged 65 and older has AD. In Wisconsin, this number is expected to increase by 58% from 103,000 to 163,000 persons over the next 25 years.

19 million Americans say someone in their family has the disease.

50% of persons with AD are undiagnosed and therefore, untreated.

Families delay seeking help for almost three years after the onset of cognitive symptoms.

50% of nursing home residents have AD or a related disorder.

Family and friends provide almost 75% of home care. Costs for at-home care average $12,500 each year with families paying almost the entire cost out-of-pocket.
